Some 26 major public inquiries have made 600 recommendations over 13 years
The National Audit Office said the probes had cost almost £240million 
But it said the Government made little effort to show it had learned from them 
Inquiries included a probe into Iraq, investigations of newspapers and a look at how Alexander Litvinenko was murdered by Kremlin agents in London
